The Long Term Home Health Care Program provides individualized, compassionate care in your home.
Do You Know Someone Who
Has a chronic condition requiring nursing supervision
Has difficulty following medication or treatment plan
Has frequent hospitalizations or trips to the emergency room
Is under a Doctor's care
Resides in Brooklyn
Has active Medicare or Medicaid coverage/eligible for Medicaid
Requires a minimum of (2) two services that the program provides
